PERSONAL PHYSICIANS continue to actively participate in the patient's care including writing orders and consulting with the hospice team. MEDICAL directors/HOSPICE PHYSICIANS offer their expertise in pain and symptom management and oversee the patient’s hospice care. NURSES manage patients' symptoms, educate patients and caregivers, and coordinate care with the rest of the hospice care team. CHAPLAINS offer spiritual care by wishes of the patient/family, often coordinating with your clergy or religious leader. SOCIAL WORKERS conduct assessments, and provide counseling, education, and other skilled interventions to enhance patient and family coping. CERTIFIED NURSING ASSISTANTS/Home Health Aides & Homemakers provide personal compassionate treatments such as bathing, nail and skin care, and grooming. Prepare meals, change bed linens, straighten room, laundry, and more! BEREAVEMENT COORDINATORS offer grief support and counseling to patients, families, and caregivers. Services are offered throughout hospice care and are followed by 13 months of bereavement support. TRAINED VOLUNTEERS provide companionship and a calming presence to the patient. They may also offer short periods of respite to the caregiver to allow them to run an errand or keep an appointment.
Program Fees: By Your Side Hospice is a service available to all with terminal diagnoses/illnesses.
Eligibility Requirements: Clients must be in need of Hospice care .
