Description:
This ADA recognized program provides comprehensive diabetes education taught by a knowledgeable multidisciplinary team including a Diabetes Educator (RN), dietician, and pharmacist. This physician-referred service is appropriate for anyone who has uncontrolled diabetes, is newly diagnosed, has had a change in medications, or has recently been hospitalized for diabetes. It is for newly diagnosed patients as well as people who have had it for years and are now struggling to control it. Both individual and group instruction is provided and Medicare and most insurances will cover some or all of the cost.
