Description:
Sight for Students is for children under 18 years old who have not graduated from high school. We provide families with information on vision programs.Family cannot have MA or other vision insurance. The family income can be no more than 200% or the Federal Poverty Level. Must be a US citizen or have a parent who is a citizen or a documented immigrant with a social security number and not have used Sight for Students in the last 12 months. Sight for Students provides a free eye examination and also provides free eye glasses. For futher information about eligibility & to get an application, contact Kim Trumm at 608-935-3379 or at the Head Start Center in Dodgeville, 212 E Chapel St, Dodgeville, WI 53533. If your child is found eligible, you will receive a gift certificate good for care from the particiapting eye doctor of your choice. You will set up the appointment.
