APPLICATION/REFERRAL PROCEDURE
An application form may be obtained from local chapters. You can call or email also to request a form. The form is to be completed by the patient or legal guardian. It must include the diagnosis and be signed by the attending physician.

Diagnosis of leukemia, myeloma, lymphoma, or other blood -related cancer

GROUPS PROVIDED/PURPOSE OF GROUPS
Family Support Groups: Guided by two healthcare professionals, the groups are intended to give information and support and to encourage greater communication among patients, their families, freiends and healthcare providers.
*Madison- Group meets the second Tuesday of each month at the UW West Clinic 5:30-7:00PM for Lymphoma/Leukemia
*Brookfield- Group meets the third Thursday of each month a LLS Brookfield office 6:30-8:00PM for Lymphoma/Leukemia
*Brookfield-Group meets the fourth Wednesday of each month at 4:00-5:30PM for Myeloma

SERVICE DESCRIPTION
First Connection - connecting survivors with newly diagnosed persons.
Support groups - offered to patients, their families and friends.
Educational Materials / Free Symposiums - offered to anyone.
Financial assistance up to $500 a year per person is given by the society to patients being treated for leukemia, lymphoma, multiple myeloma and other blood-related cancers.
Financial assistance up to $500 for out of pocket medical and travel expenses for family members and patient.
Services covered: drugs, processing, fees for transfusions and transportation.
